I/my child, Participant named above, agree to participate in cardio/fitness or strength classes with a certified Cam Fitness, LLC Training Instructor. I/my child recognize exercise is not without varying degrees of risk to musculoskeletal and/or cardio respiratory systems. I hereby certify that I know of no medical problems that would increase my/my child's risk of illness and injury as a result of participation in a fitness program designed by Cam Fitness, LLC. I/my child understand and have been informed that there exists the possibility of adverse changes during the exercise program. I/my child have been informed that these changes could include abnormal blood pressure, fainting, disorder of heart rhythm, stroke and very rare instances of heart attack of even death. I/my child agree to waive, release, remise and discharge Cam Fitness, LLC and its agents, Officers, Principals and employees of any and all claims, demands, actions or damages of any kind resulting from participation in Aerobic Kickboxing, Strength, HIIT and Cardio conditioning classes or individual training sessions. The undersigned hereby releases Cam Fitness, LLC as well as waives any and all claims and understands and assumes any and all risk with participation in Cam Fitness, LLC programs.

In consideration of good and valuable consideration, the receipt and sufficiency of which is hereby acknowledged, I, the undersigned, hereby grant to Cami Olson/Cam fitness ("Trainer") and his/her agents the right to use my name, biographical information, photographs, images, story and/or testimonial, in whole or in part, and without restriction as to changes or alterations. the rights granted herein shall extend in perpetuity, unless revoked in writing to Trainer by me, throughout the world and for any purpose whatsoever, including without limitation for marketing and advertising purposes of Trainer, and in any and all media, including without limitation Trainer's website. I acknowledge that trainer has no obligation to return any photographs or images to me.
I hereby RELEASE, WAIVE, and FOREVER DISCHARGE any and all claims arising out of, or in connection with, such use Trainer, including without limitation any and all claims for libel or invasion or privacy.
I hereby warrant and represent that I am at least 18 years of age and have the right to contract in my own name. I have read the above Release and am fully familiar with the contents thereof. This Release contains the entire agreement between the parties hereto as to the subject matter contained herein.
Corona Virus Waiver
By entering this facility, you are aware that you agree to fully accept all known and unknown risks, including the potential risk of exposure to respiratory illnesses such as the coronavirus (COVID-19). The coronavirus is primarily transmitted via exhaled respiratory droplets, most often through coughing and sneezing. These droplets can travel up to six feet and are more commonly transmitted between persons rather than from equipment to persons.
Although we regularly sanitize our equipment and presently are using enhanced cleaning methods and enforcing social distancing in our facility, you understand that you may be exposed to the coronavirus or its symptoms through no fault of our own. Known coronavirus symptoms include fever, coughing, shortness of breath, pneumonia, kidney failure, and may include other symptoms, stroke or even death (collectively "Symptoms"). You understand and agree that you will hold us harmless and you will not hold us liable for any real or perceived Symptoms of COVID-19 or any other disease, illness, or condition, nor for exacerbating any existing symptoms, and you fully agree to accept all risks of entering the facility, using the equipment, working with personal trainers, attending classes, and/or interacting or being exposed to other members.
